Your pet's name, your name, address and phone number. Please send at least three photos of your pet to me at printsopaws@gmail.com. At least one head shot and one body shot. I cannot process your request without photos(required)

Please provide the following info: Type of pet, age, weight, breed, female or male, spayed or neutered, current on vaccinations, current on heart worm prevention?(required)

Please describe your pets appearance, such as color and coat length, etc. Please tell us about your pet's personality, overall health, their eating and sleeping habits, indoor or outdoor pet or both.(required)

How does your pet respond to: other pets or animals, small children, strangers? What, if anything, is your pet fearful of, for example: lightening, loud noises, etc.(required)

Is your pet house trained, crate trained, leash trained? Is your pet ok on car rides? Please describe any issues(required)

Please describe any behavioral issues you pet may have, for example: food or toy aggression(required)

Please tell us about any health issues your pet may have, for example: daily medications, medical conditions, etc.(required)

Are you willing to have your pet spayed/neutered before they are adopted?(required)

Please share with us, in more detail, why you need to re-home your pet and WHAT DATE YOU NEED YOUR PET RE-HOMED BY. If you do not hear from us within two business days please contact Lori directly at printsopaws@gmail.com.(required)
